While the typical massage client used to have to track down a freelance massage therapist, schedule an appointment days in advance, and pay over $100 for a one-hour treatment, most Hand & Stone clients book their appointments on the same day as service and pay 30 percent less than the traditional hourly rate. Better yet, when they sign up for a one-session-per-month plan, they end up paying around $60 per visit.

As massage spas become more mainstream in the health care industry, Hand & Stone stays at the top of the game by adding locations and services to keep the brand fresh. Chemical Peels, as the words suggest, are usually used to peel the outer layers of the skin. They are also called exfoliates and are commonly used when dealing with acne on the skin, mild sun damage, wrinkles as well as other such things. The tattoo pigment is located below these layers of skin so it is almost impossible to remove it by using chemical peels.

The active ingredient in chemical peels used for tattoo removal purposes is TCA (TriChloroacetic Acid). So in essence the TCA will burn away the outer layers of the skin in order to allow the skin to form new layers in their place. Depending on the amount of time the chemical peel is left on the skin will determine how many layers have been peeled off. They range from a superficial peel to a deep peel, all of which have different healing times and aftercare treatments.

What are they most commonly used as?

The main 2 reasons why people will use chemical peels will be acne and aging. Uses include:

WebMD as well as other such medical websites do not mention anything about using chemical peels as a tattoo removal method because they will simply not work. The guidelines state that:

Deep peels are not done on darker skin types, because they bleach the skin.

This is how a chemical peel masquerades as a tattoo removal method as it bleaches the skin and thus makes it seem like the tattoo has been removed when in fact it is the skin that has been lightened and gives off a “fading” effect on the tattoo.

Nationwide television commercials and magazine ads abound with over-the-counter (OTC) products. Although largely untested in controlled clinical trials, many of these products are considered somewhat effective, particularly for patients with mild acne. Those which have been tested include salicylic acid-containing topical products and many benzoyl peroxide (BP) products described in further detail later. Salicylic acid has revealed some efficacy in acne trials, although when tested head-to-head with other topicals, particularly BP, it is generally less effective.22,23 Nonprescription, nonbenzoyl-peroxide-containing products appear to be somewhat effective for the treatment of acne, especially mild acne, though there is limited published evidence supporting their efficacy in the treatment of acne.

Sulfur, sodium sulfacetamide, and resorcinol are active ingredients in several OTC dermatology niche products. Sulfur exhibits mild antibacterial and keratolytic properties.24 Because of sulfur’s distinctive odor, it is often combined with sodium sulfacetamide to mask the scent.25 It is often used in adult female acne because of its favorable tolerability.26,27 Resorcinol also has mild antimicrobial properties and is typically formulated in a 2% concentration in combination with 5% sulfur.

One common acne myth is that poor hygiene and improper cleansing cause acne.21,28 The role of facial cleansing in acne is to remove makeup, dirt, and excess oil.29 Use of the wrong, too harsh cleanser can disrupt skin barrier, increase transepidermal water loss, encourage bacterial colonization, promote comedones, and cause symptoms of burning and stinging.30,31 Typically, twice-daily washing with a gentle soap-free, pH-balanced cleanser is recommended. Antibacterial washes, other than BP, have not been shown to be useful in the treatment of acne.

Facial toners can decrease oiliness and remove makeup and traces of dirt. They are a common component of several prepackaged combination acne treatment regimens. Patients should be cautious not to overuse facial toners because they can be irritating. If irritation occurs, this will adversely affect the tolerability of acne medications.

Another common acne myth is that use of cosmetics worsens acne. On the contrary, use of concealing oil-free, noncomedogenic makeup can improve patient quality of life and does not worsen the severity of acne.32,33 Use of cosmetics in patients with acne has not been shown to delay treatment response either.

BP has been shown to be the most widely studied of OTC products and has shown to be one of the most versatile, safe, inexpensive, and effective acne therapies.34,35 Its lipophilic nature permits it to penetrate the stratum corneum and enter the pilosebaceous unit where P acnes resides. It acts via the generation of free radicals that oxidize proteins in the P acnes cell wall. It also has been shown to have mild comedolytic36 and antiinflammatory properties.37,38 BP helps limit the development of P acnes resistance to antibiotics and also provides increased efficacy in combination with retinoids.39,40 So far, antibiotic resistance to BP has not been reported.41-44

Although issues regarding genotoxicity have been raised in the past, BP has now been labeled as “GRASE” (generally regarded as safe and effective) by the FDA, and all topical monotherapy products have been made available OTC since 2011. Labeling includes advice to avoid the eyes, lips, and mouth. The product can cause bleaching of hair and clothing, and risk of increased sunburn and the need for photoprotection also are mentioned. BP frequently causes dryness, erythema, and peeling upon initiation of treatment. Starting with lower concentrations (eg, 2.5%) and utilizing more emollient vehicles if needed can help alleviate these discomforts. Allergic contact dermatitis to BP occurs in 1 in 500 people and should be considered if a patient complains of itching and swelling of the eyes.

BP is available in a variety of formulations and in concentrations ranging from 2.5% to 10%. There is some evidence that higher concentrations do not increase efficacy but are more irritating. However, the back may be a “special site” circumstance, where increasing concentration or prolonged contact leads to increased efficacy.45 Formulations include a variety of topical leave-on preparations as well as washes that permit patients to remove BP from the skin, reducing the possibility of bleaching of clothing, bedding, or towels.38 It has been suggested that short-contact BP therapies do not significantly reduce bacterial load, but data are lacking. However, they can be effective if left on the skin for the duration recommended by the manufacturer.

Consensus Recommendations:

BP is generally regarded as a safe and effective medication that may be used as monotherapy or in topical combination products for mild acne or in regimens of care for acne of all types and severities. (SOR: A).

BP may minimize development of antibiotic-resistant P acnes when used with topical or systemic antibiotics. (SOR: C).

Our skin is the largest organ of our body. As we age, dead skin does not slough off as it should, resulting in a dull, uneven toned and bumpy appearance. This inhibits the skins ability to eliminate toxins. This layer of dead skin must be removed. Dead Sea salt combined with the natural glycolic acid in sugar provides the necessary abrasive action to re-textures the skin giving it a smooth, soft feel.

Dead Sea Salt is particularly therapeutic because it is known to help remove toxins and is traditionally less drying than regular salts. They contain many minerals; including magnesium, zinc, calcium, potassium, and bromide as well as many other trace minerals. It aids the skin in detoxification as it removes dead skin. The latter is important, as the body looses these important trace elements through perspiration. Also sea salts that have different grain sizes that are optimal since the larger grains whisk away the dry skin and the smaller grains provide a polishing effect to produce the smoothest texture.

Will the hairs grow back thicker on my face?

No. It is physiologically impossible for your vellus hairs to grow back thicker. True, once the hair is cut, it has a blunt edge. But, this blunt edge does not mean that the hair structure itself has been physiologically altered.

Vellus hair when cut or removed will grow back the same. The structure of the hair does not become damaged, therefore it is impossible to alter.

Blading the skin is performed on the face only. Men are also candidates for this treatment; due to the nature of course hair growing in the beard area, men can only be treated on the upper cheeks, around the eyes and forehead.

How often is the procedure performed?

Blading can be performed every 3-4 weeks. Blading the skin actually removes about 2-3 weeks worth of dead skin cells. We want the skin to complete its normal skin cycle of approximately 30 days.

There is some downtime with these treatments, and Dr. Capone administers these peels personally to ensure great results and a speedy recovery.

Aftercare Instructions:

Depending on the depth of your chemical peels, there are varying degrees of associated aftercare, and our skin care team will provide you with specific aftercare instructions. In general, it is ok to wash your face with very gentle cleanser the day after a peel, but NEVER scrub or pick at flakiness. For superficial chemical peels, begin wearing sunscreen immediately after your peel and avoid direct sun exposure for 4 to 6 weeks. For deeper chemical peels, you won’t be able to use sunscreen initially so limiting sun exposure is a must. (Usually sunscreen can be resumed by about 1 week.) Peel patients should avoid hot showers, saunas, and heavy exercise until cleared by your aesthetician. If scabbing does occur, keep the area moisturized and inform our medical aesthetician. Never pick or rub. We will let you know when it’s appropriate to resume use of your regular skin care regimen.

What Can I Expect from Chemical Peels?

We offer four chemical peels at our medical spa.

Alpha Hydroxy Peels (AHA) leave the skin smoother and even-toned. Because of their milder nature, they are a good choice for those who desire little downtime. They also are recommended for those who have darker skin tones, since they are less likely to cause a lightening of the skin. These peels are available in different strengths, with peeling time varying accordingly. Flaking, if it occurs, may last a couple days but all patients heal differently. Best results are usually achieved by undergoing a series of treatments.

The BioMedic MicroPeel® Plus is designed to improve skin that is acne prone and/or environmentally damaged. Its key ingredient, salicylic acid, is a beta hydroxy acid that exfoliates dead skin cells and other debris that can clog pores and lead to blemishes. Skin is left smoother and more refined. This peel is available in two concentrations depending on the level of treatment you desire. Any flaking that may occur usually begins a few days after the peel is applied and lasts for a few days afterwards; however, all patients are slightly different in how they react and heal. Because this is a lighter peel, best results are usually achieved by having a series of peels, with approximately two to six weeks between each treatment.

The Obagi® Blue Peel uses TCA (trichloroacetic Acid) mixed with a blue solution to remove the damaged outer layers of the skin. The new layer of skin that comes to the surface will appear tighter, smoother, and more evenly toned. ACPS offers the doctor-administered TCA peel as an alternative to the Obagi® Blue Peel.

A TCA (trichloroacetic acid) peel is a much stronger peel than the AHA or BHA peels and is administered by Dr. Newall. TCA diminishes such skin imperfections as fine lines, sun damage, and uneven pigmentation by sloughing off the damaged surface layers of the skin so that a fresher, more youthful-appearing layer of skin can emerge. Dr. Newall will adjust the strength and depth of the peel based upon your individual needs and desires. The healing period for this peel varies depending on how much is applied and your individual healing response, but typically takes a week to 10 days.

Am I a Good Candidate for Chemical Peels?

Chemical peels may not be right for you if you are pregnant or breastfeeding. People with certain skin conditions should not have chemical peels, and Dr. Newall will recommend the best alternative treatment.

As a skin treatment method, microdermabrasion is quite forgiving. It can deal with various skin issues with hardly any hassle or any kind of dangerous outcomes. Acne is recognized as the number one skin problem in the world with teenagers as its more common victims. Acne is not limited to teenagers, but it can occur to people of all ages. Infants or babies are not any exceptions to that.

Causes of Baby Acne

Baby acne, also known as infantile acne, is not uncommon. Following are the causes of infantile acne:

Excessively Oily Skin
Although infants usually don’t suffer from oily skin, however, in rare cases, baby acne is caused by excessively oily skin. If you find that your bundle of joy has oily skin and has acne, you should wash the skin of your sweet little baby with warm water and mild baby soap at least three times a day.

Bad Nutrition
Since your baby cannot pick up bad eating habits at this early age, infantile acne due to bad nutrition is very rare. Bad nutrition can be the cause of infantile acne if parents are not attentive toward their babies’ diet. If you are thoughtful of your baby’s diet, then what is causing your baby to suffer from acne is —

Change of Hormones
It can happen to infants before their births. Mother’s hormones get passed to the baby via the placenta. Change of hormones can also happen when the baby has been separated from the mother. Mother’s milk is also a source of hormones for the baby.

Due to any of the above mentioned causes of baby acne, acne can occur to babies in as young age as 2 to 3 weeks.

Baby Acne Treatment
Many doctors suggest not using anything for baby acne, but just mild baby soaps with warm water on the baby’s skin during baths for cleansing the skin in order to treat baby acne. You should do just as the doctors suggest. However, if acne doesn’t disappear by the advised cleansing method, you can try, after consulting from your baby’s doctor, very mild over-the-counter 100% natural acne treatment lotions or creams.

It is important to distinguish between dermabrasion and microdermabrasion. Dermabrasion is a much more invasive procedure performed using special sanding devices. Dermabrasion causes as much or more damage to the skin as laser resurfacing or deep chemical peels. The skin recovery after dermabrasion takes several weeks or more. Dermabrasion has been used in cosmetic surgery for decades. However, its use has been declining since the advent of laser resurfacing. Microdermabrasion, on the other hand, is only a mildly invasive procedure. It causes virtually no downtime and the patients can usually return to their regular activities the same day. In fact, some call microdermabrasion a lunchtime peel. Convenience and minimal recovery period account for much of the popularity of microdermabrasion. Some people are also enticed by seemingly low cost of a microdermabrasion treatment (usually in the range of $100-$300 per session). Is there a catch? Unfortunately, there is. First, the effect of microdermabrasion is generally modest. Some people see no effect whatsoever. Others see a small effect, and only a relatively small percentage of people report substantial benefits. Actually, this is understandable: a mild mechanical treatment like microdermabrasion is unlikely to cause major skin restructuring required for a dramatic reduction in wrinkles. Repeated treatments may provide cumulative improvement in some cases, but often produce little or no benefit. (And the cost is cumulative whether you get cumulative benefits or not). Since its effects tend to be modest, microderabrasion is generally believed to be more useful for early signs of aging and minor skin lesions. In particular, it is often recommended for mild acne scarring, uneven pigmentation, poor skin texture, fine lines and dull skin. Unfortunately, clinical research substantiating the benefits of microdermabrasion is somewhat scarce. In one study published in the journal Dermatol Surgery in 2001, Dr Freedman and colleagues from Plastic Surgery Associates of Northern Virginia analyzed the onset and extent of the dermatologic changes associated with microdermabrasion. Ten volunteers, ages 31-62 years, underwent a series of six aluminum oxide microdermabrasion facial treatments 7-10 days apart. The researchers reported improvement compared to the control group: Compared to the controls, the treated areas demonstrated the following histologic changes: thickening of the epidermis and dermis, flattening of the rete pegs, vascular ectasia and perivascular inflammation, and hyalinization of the papillary dermis with newly deposited collagen and elastic fibers…. This study suggests that microdermabrasion produces clinical improvement by a mechanism resembling a reparative process at the dermal and epidermal levels.
Notably, microderabrasion appears to enhance the effects of topical treatments. In fact, when microdermabrasion is combined with topical antioxidants (and perhaps other topical agents), the benefits appear to be synergistic. In another study, published in the Journal of Dermatological Treatment in 2009, Dr Freedman from Plastic Surgery Associates of Northern Virginia investigated whether the addition of a polyphenolic antioxidant serum enhanced the dermatologic changes seen following microdermabrasion. Ten female volunteers, aged 38-52 years, underwent a series of six microdermabrasion facial treatments spaced 7-10 days apart. An antioxidant serum rich in polyphenols was pneumatically applied to half the face immediately after each microdermabrasion treatment. I decided to go for microdermabrasion at a clinic as I found out that it is cheaper than some ridiculously priced salon microdermabrasion facials. I once asked a beautician why microdermabrasion at their spa is so expensive, more expensive than medical microdermabrasion at the clinic and she gave me a silly ‘relaxation and service’ excuse.

What I want is a good medical grade microdermabrasion to thoroughly scrub off my layer of dead skin cells and renew my skin, not (lousy) service that most spas provide. My last few facial trips to different spas have been thoroughly disappointing, with me breaking out after the facials. So I decided I would go to a clinic instead, and as my nursing friend put it, if anything happens to your face, there is always a doctor there!

When I first entered the treatment room, the nurse put a pillow behind my back and another at my neck, then the chair was lowered to almost horizontal. She then covered me with a blanket cos the treatment room is cold. (The service is similar to salons and the nurses there are friendly). They offered me drinks: coffee,tea or water and some biscuits while the nurse prepared the machine as it was my ‘lunchtime procedure’.

The nurse double cleansed (I think she used Cetaphil, just my guess!). Then the microdermabrasion started! Unlike salon microderm where it feels like the machine is vacuuming/sucking your skin, the clinic one was more painful. It felt more scratchy, a bit like sand-papering your skin – which is of course the intended purpose, to remove the surface epidermis so that the underlying skin is stimulated to produce collagen and newer skin beneath is revealed. So definitely more intense and deeper removal of skin than the spa microdermabrasion. It was painful for me, especially around the area around the eyes. She concentrated the wand at the t-zone and my cheek area where I had more clogs.

Unlike spas, where they just do one round of suction, the nurse made several rounds around my face using the rougher wand, then followed up with a lighter wand which didn’t hurt, then ended with an even gentler one for the eye area. So the entire process was very thorough and covered pretty much every part of my face and also part of the neck area.

After the procedure, the nurse will put on a mask for 15-20mins then clean it off and apply moisturiser and sunblock for you. She used La Roche-Posay moisturiser and Heliocare SPF90 gel sunblock.

For me, this procedure was painful and my face was red for about 2 hours and I felt the sting the rest of the day. At around evening time when I washed the sunblock off, my skin became red and stinging again. I think it was cos my skin type is sensitive thats why it was red and stinging for so long. It did not sting that much for my friend but for her, her skin was very dry after the procedure and peeled a bit.

The next day, the sting and redness was gone and my skin looked more even toned, brighter and felt smoother. The blackheads on my nose were gone! But they came back after a few days, which I presume is normal. The red pimple marks were also lightened. I was happy with the results as it seemed more worth it compared to spending big money at spas that aren’t that thorough and use a less effective machine. I think I will go back and do microdermabrasion again, since the effects were quite good for me and it seemed more effective than a normal facial.

**Edit: I did not include the clinic name as this post is meant to share my experience of doing microdermabrasion at a clinic as compared to a facial salon/spa. Jessner’s and Trichloroacetic Acid (TCA) peels

Jessner’s and TCA peels are medium depth peels designed to improve more extensive wrinkling, sun damage, uneven pigmentation, and acne.

A Jessner’s peel is made from a mixture of salicylic acid (a beta hydroxy acid), lactic acid (an alpha hydroxy acid) and resorcinol. This is the lighter of the two medium peels and is often used alone or sometimes as a pre-treatment to a TCA peel. A Jessner’s peel is meant for moderate sun damage, pigmentation, wrinkling, and acne scarring. There is commonly redness of the skin for 2-3 days and then peeling like a sunburn for 2-3 days. There is commonly moderate swelling and crusting of the skin for about a week, peeling can be expected for up to two weeks. Patients can often return to work and apply makeup in 7-10 days.

Microdermabrasion is a popular, safe and gentle cosmetic procedure that mechanically exfoliates the skin. This technique uses a closed pressurized system, designed to improve the appearance of fine wrinkles, skin texture and colour by polishing the topmost layer and then vacuuming away the dead skin cells in a safe, controlled manner.
